We are happy to help you obtain a copy of a baptismal or marriage certificate from St. Luke’s Church. Please complete the form below and provide as much information as possible so we can locate the record in our parish archives.
For Weddings or School Registration

There is no fee if the certificate is required for a wedding at St. Luke’s Church or for registration at our parish school.
For Canadians
Processing fee: $50 CAD

To help us locate the correct record in our parish archives, please provide as much information as possible when completing the form.
Helpful details include:

Full name of the person at the time of baptism or marriage

Approximate date of the baptism or marriage

Parents’ names (for baptism records)

Any previous or alternate spellings of names

The reason the certificate is needed (for example: marriage, citizenship, school registration, genealogy, or personal records)

For requests from Canada, a $50 CAD processing fee is requested to help cover the time required to search parish records and prepare the certificate. Payment may be made by e-transfer to office@stlukesdowneyville.ca or cheque to 340 St. Luke's Rd, Lindsay, ON K9v4R5
